Group medical insurance coverage begins the first of the month following thirty days (30) from the date of hire. Should you waive medical coverage at the time of hire you may only enroll during the next "open enrollment" period. If other medical coverage is lost or there is a change in status due to divorce, marriage, birth or adoption of a child, you may be eligible fora "special enrollment".
Upon eligibility, insured employees will receive identification cards and booklets describing the coverage. Benefit coverage will cease the end of the month in which a termination occurs unless employee qualifies and elects to continue coverage at his/her own expense through COBRA.
An employee contribution towards the monthly premium will be made through a pre-tax payroll deduction each pay period.
