Vision Insurance
Group vision coverage is available at the employee's expense. Vision premiums are deducted from payroll on a pre-tax basis. A two year "lock in - lock out" is required.
Employees are eligible for coverage the first of the month following thirty days (30) from the date of hire. Should you waive vision coverage at the time of hire you may only enroll during the next "open enrollment" period. If other vision coverage is lost or there is a change in status due to divorce, marriage, birth or adoption of a child, you may be eligible fora "special enrollment".
